First Visit: What to Expect

New Motion Physical Therapy (NMPT) is a physical therapy clinic located on Bainbridge Island WA that is dedicated to rehabilitation achieved through the use of manual therapy, Pilates based exercises and various other treatment techniques.

What to expect:

  • On your first visit, your therapist will review your medical history form with you, evaluate the condition for which you were referred, develop and implement a treatment plan.

What to bring:

  • Bring your prescription, insurance card, and relavant co-pays.
  • In order to maximize our time with you on your first visit, we have provided you with forms to be printed off and brought with you to your first appointment. The information in this registration packet assist us to learn about the history of your current condition and any other pertinent medical knowledge which affect your daily life. In addition, it allows us to accurately bill your insurance.
  • Payment for the portion of the visit you are responsible for. 

What to wear:

  • Please wear comfortable clothing that allows access to the area being treated.
  • If you have a lower extremity or low back injury, shorts may be required.
  • Athletic or supportive shoes are needed to effectively perform many different exercises while at the clinic. Please avoid wearing flip-flops or high heels.

Communication

  • Your therapist will update your health care provider with a written status report every 30 days, or more often if needed or requested by your referring provider.
  • Your privacy is important to us. If you would like your therapist to communicate with an outside source, a release of information form must be filled out and signed.

Financial Information

  • Patients are financially responsible for services provided to you
  • Deductibles, Co-insurance, and Co-pays are due at the time of service
  • We will credit/bill you for any difference after your claims have been processed.
  • Payments on account  are due by the 30th of each month.
  • Cash, checks, Master Card, and Visa are accepted.
  • Late fees are applied to past due accounts.
